Residential Setting

The impact of Residential Settings is to provide individuals the ability to live in a safe, stable, least restrictive residential setting of their choice. A Residential setting is a place to live where an individual has services and supports, ranging from 24-hour supervision to on-call assistance, to live as independently as possible. These services will allow an individual to feel that they are a part of the residential community.

Following are the goals that allow individuals to:

  • move to a less restrictive residential setting
  • provides, a safe, more stable residential setting
  • transfer from a homeless environment or shelter to a more stable residential setting
  • receive support that minimizes at-risk situations for individuals with a brain injury
Goals Exceeds Expectation Meets Expectation Below Expectations
RS1: Individual moves to a less restrictive residential setting. Greater than or equal to 75% of individuals who have a personal goal to move to a less restrictive residential setting achieved this goal by the target date. 50-74% of individuals who have a personal goal to move to a less restrictive residential setting achieved this goal by the target date. Less than or equal to 49% of individuals who have a personal goal to move to a less restrictive residential setting achieved this goal.
RS2: Individual in an at-risk situation moves to, or receives supports that provide, a safer, more stable setting. Greater than or equal to 75% of individuals who have a personal goal to move from an at-risk situation to a safer, more stable residential setting achieved this goal by the target date. 50-74%  of individuals who have a personal goal to move from an at-risk situation to a safer, more stable residential setting achieved this goal by the target date. Less than or equal to 49% of the individuals who have a personal goal to move from an at-risk situation to a safer, more stable residential setting achieved this goal by the target date.
RS3: Individual who is homeless or in a shelter moves to a more stable residential setting. Greater than or equal to 75% of individuals who have a personal goal to move from a homeless / shelter situation to a more stable residential setting achieved this goal by the target date. 50-74%  of individuals who have a personal goal to move from a homeless / shelter situation to a more stable residential setting achieved this goal by the target date. Less than or equal to 49% of individuals who have a personal goal to move from a homeless / shelter situation to a more stable residential setting achieved this goal by the target date.
