OUTCOME: Individuals live in a safe, stable, least restrictive residential setting of their choice.
DEFINITION: Residential setting is a safe and stable place to live where an individual has services and supports, ranging from 24-hour supervision to on-call assistance, to live as independently as possible.
RESULTS: Of the total number of individuals who have a personal goal to change their residential setting, what percentage achieved this goal by the target date?
